ACRA Vic/Tas Workshop
This afternoon workshop is designed for both acute and outpatient cardiac nurses and will dive deep into the link between Phase I (inpatient) cardiac rehabilitation and Phase II (outpatient) cardiac rehab; Patient education, including how to provide education in busy inpatient clinical environments, consideration of health literacy, use of the Teach Back method and available patient resources; How to strengthen referral process and write a good referral.
