Cardiac Rehabilitation Program

Cardiac Rehabilitation is an exercise and education program which is for people suffering from cardiovascular disease.  Cardiac rehabilitation, consists of three phases.

Phase 1

This phase consists of inpatient education.  This may be delivered by a nurse or an allied health professional following heart surgery or a cardiac medical procedure.  It includes providing information to the patient regarding managing cardiovascular disease, secondary prevention, diet, exercise, medications, social and emotional issues as well as the benefits of attending a cardiac rehabilitation program.

Phase 2

This phase consists of both exercise and education and usually lasts for about 4-8 weeks.  The exercise component may be delivered by an exercise physiologist, physiotherapist or a nurse and may take place in a hospital gym setting or out in the community.  The education component visits a range of topics such as diet, exercise, medications, social and emotional issues and returning to activities such as work, mowing the lawn and driving.  The education program is delivered by the multidisciplinary health professional team.

Phase 3

Phase 3 is the maintenance exercise program and starts after completion of Phase 2.  This may take place within a formal cardiac rehabilitation program or it may be a self-motivated home exercise program.  For those who have moved on from the formal cardiac rehabilitation program, phone support with these programs is still usually available for any concerns or questions.
